Post-Acute Neurorehabilitation – Supported Living – Day Treatment
HealthBridge’s Residential Program located in St. Louis (on the same campus as our post-acute care and rehabilitation program) provides a beautiful setting for recovery, offers gourmet meals, and allows its residents to surround themselves with their own belongings in a private room. For those who no longer require around-the-clock nursing or skilled services, our program offers solutions from various levels of neurorehab to supported living to semi-independent living, and even day treatment programs. With a high staff-to-patient ratio and 24/7 care, we provide an environment conducive to quicker community reintegration.
HealthBridge’s campus offers a full continuum for brain injury and spinal cord rehabilitation allowing patients in need of specialty nursing to be treated at our adjacent facility, while still receiving treatment from the same therapy team.
At HealthBridge, we believe that every person, through individualized treatment plans, personalized attention and intensive treatment has the opportunity to improve their quality of life and live at their highest level of independence.
HealthBridge is a leading provider of complex rehabilitation and transitional care services for patients of all ages in a family-friendly environment. We are able to meet the needs of every resident we serve through 24/7 care, a superior collaborative treatment team, specialized equipment, and proven clinical and therapeutic protocols.
Residential Rehab Program
In addition to our medical post-acute care and rehabilitation program in St. Louis, we also offer a residential rehab program for injured workers who have sustained a neurological injury as a result of trauma or other related medical events. HealthBridge offers a fully integrated home and community program to facilitate the transition to home and, in some cases, return to work.
These patients are not ready to live independently, but do not require the skill level for care and rehabilitation offered at a post-acute facility. In addition, they are not bed-bound, and during the day find themselves involved in a variety of activities and interacting with other residents facing similar challenges. Our treatment team assesses the needs of each individual resident and develops a targeted program centered around the unique services required for each resident.
