HealthBridge’s post-acute care facility focuses on complex care and rehabilitation, spinal cord injury recovery, traumatic brain injury long-term care, and medically complex cases, such as patients with neuromuscular disorders (such as ALS, Parkinson’s, MS, and Guillain-Barré), pulmonary disease or injuries, polytrauma, congenital anomalies, and multifaceted wounds. Our patient services are advanced, and include ventilator care, trach care, vac therapy, halo care and more, reducing the need for extended, expensive acute care hospital stays.

Measuring progress can be seen in a variety of ways, ranging from a patient’s ability to breathe on their own, to move a hand or to operate a power wheelchair. Improvement may also be evident as a patient receives training to function independently in a comfortable environment.

In addition, we use many standardized assessment tools such as the Rancho Los Amigos scale to help track stages of recovery. The Rancho scale (named after the Rancho Los Amigos National Rehabilitation Center), assesses the level of recovery of brain injury patients and those recovering from coma.

Upon leaving an acute care facility, a medically fragile patient will probably require further medical treatment during the recovery process, but is no longer in need of intense medical intervention. These types of patients may include those with medically complex illnesses, pulmonary or cardiovascular disease, in addition to those with neurologic disorders.

The emphasis at a post-acute care facility is on rehabilitation and symptom management. While there may be some level of medical care required, it’s likely to be moderate with the focus on recuperation and rehabilitation with the goal of providing the patient with the highest optimal level of independence.
