Activity Leader

Orange, CA (Orange County)

Activities Leader is under the direction of the CCO and Nurse Manager. This position provides oversight of the facility activities and volunteer program.

Responsibilities:

Completes initial assessments and initiates patient activity program. Obtains additional information from medical record and family members. Follows any medical regime developed for patient while ensuring awareness of precautions (cardiac program, infection control, pressure relief, bed rest limitations) and reports any changes in patient’s condition to Charge Nurse immediately. Develops a therapeutic relationship with patients. Coordinates with other patient services in scheduling of therapeutic activity time. Plans appropriate activities for assigned group.

Qualifications:
  • Activity Leader Educational Certificate- Long Term Care
  • Activities/Long Term experience preferred

Certified Nurse Assistant (CNA)

Orange, CA (Orange County)

Under the direct supervision of the nurse, the Certified Nursing Assistant assists in the direct patient care and assists patients with activities of daily living.

Responsibilities:

Assist residents/patients with personal hygiene and care such as bathing, grooming, and dressing, and bowel and bladder care. Take and record patient vital signs and weight. Observe and record patients’ conditions, food and liquid intake and other behavior, and report changes to supervisor. Maintain clean and safe environments for patients. Lift, transport, and move patients using the appropriate techniques. Treat all patients, family, and other visitors with respect, and always engage with them in a tactful manner. Comply with all safety and health regulations.

Qualifications:
  • High school diploma or GED preferred
  • Must hold current CNA certification (CA)
  • Prior work experience in a direct patient care setting or in a long-term care facility preferred

Housekeeper

Orange, CA (Orange County)

Housekeeper/Maintenance Assistant is under the direction of the Plant Ops. Supervisor. This position will be responsible for ensuring that a clean and sanitary environment is maintained throughout the facility.

Responsibilities:

Performing a variety of cleaning activities such as sweeping, mopping, dusting and polishing. Ensuring all rooms are cared for and inspected according to standards. Protecting equipment and making sure there are no inadequacies.

Performs environmental and plant maintenance work for a skilled nursing facility. Maintains resident rooms in good repair. Assists supervisor with maintaining all HVAC, electrical and plumbing systems in good repair and in compliance with all state and federal life and safety regulations. Assists with maintenance of current emergency action procedures and plans.

Qualifications:
  • High School Diploma or equivalent preferred.
  • Excellent working knowledge of cleaning appliances and their operations and proper use of cleaning agents.

Licensed Vocational Nurse (LVN)

Orange, CA (Orange County)

Under the direct supervision of Registered Nurse, the LVN provides direct nursing care in accordance with the laws governing nursing practice.

Responsibilities:

Take and record vital signs, such as blood pressure. Administer medications as ordered and report adverse reactions to medications or treatments. Dress wounds, give alcohol rubs, apply compresses, ice bags, and hot water bottles, bathe and dress patients, and collect blood and urine samples. Monitor patients and chart medical records accurately. Report the condition of a patient back to RNs. Communicate openly and effectively with patients and families, while demonstrating compassion and friendly behavior.

Qualifications:
  • Must be a graduate of an accredited LPN nursing program
  • Must have a valid LVN license (CA)
  • Ability to demonstrate good judgment and decision-making skills

MDS COORDINATOR

Florissant, MO (St. Louis County)

Under the supervision of the Director of Nursing, the MDS Coordinator will assure that the correct and compliant utilization of Medicare and Medicaid services is maintained. Complete accurate and timely MDS assessments for all residents. Coordinate tracking of Medicare and Medicaid data and educates facility staff on Medicare and Medicaid rules and regulations. Demonstrate expertise with PDPM. Use your excellent assessment skills to develop care plans individualized to meet residents’ needs. Routinely audit to verify facility is following Federal and State rules and regulations and established policies and procedures.

Responsibilities:
  • Educates new and current facility staff on established Medicare and Medicaid policies and procedures.
  • Audits of facility for compliance with established Medicare and Medicaid policies and procedures and rules and regulations.
  • Assists with implementing restorative nursing programs.
  • Develops and updates Medicare and Medicaid policies and procedures as changes occur.
  • Assists facility with development of Action Plans to correct issues identified through audits and develop systems to maintain compliance.
  • Works in concert with interdisciplinary team to develop individualized care plans to meet the residents needs.
  • Audits the medical records, Medicare non-coverage letters and Physician Certification and re-certification for accuracy and completion.
  • Facilitates triple check and audits a sample of the UB-04’s and compares to the medical record to verify the medical record supports the claim.
  • Monitors ADL documentation for compliance.
  • Monitors the Nursing Restorative Log.
  • Assists the facilities in Medicare and Medicaid utilization and compliance and ADR’s/Denials.
  • Participates in state Medicaid audits and is knowledgeable of state specific regulations.
  • Complete MDS per RAI.
Qualifications:
  • Maintains a current, valid license as an RN; BSN preferred, and is in good standing. Possesses strong knowledge of state, federal, and local regulations as they pertain to long-term care.
  • Possesses a minimum one (1) year of experience in Nursing Service Administration. Additional education or experience in such areas of rehabilitative or geriatric nursing is preferred.
  • Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) as it applies to clinical reimbursement.
  • Demonstrates strong knowledge of the Medicare PDPM, Medicaid and documentation guidelines.
  • Demonstrates knowledge of analyzing and tracking the Case-Mix Roster report and making recommendations accordingly.
  • Is knowledgeable of MDS coding as defined in the RAI Manual. Educates facilities on correct coding and selection of MDS assessments for payment. Expertise in PDPM.

Occupational Therapist

Florissant, MO (St. Louis County)

Under the direction of the Director of Rehabilitation (DOR), the Occupational Therapist implements high quality occupational therapy services for patients, in accordance with the state regulations, principles and practices of occupational therapy and HealthBridge policies and procedures.

Physical Therapist

Registered Nurse (RN)—Complex Care Unit Manager (CCU)

Florissant, MO (St. Louis County)

Seeking a dynamic Registered Nurse to lead, manage and direct services for our fast-paced, state of the art Complex Care Unit specializing in the care or patients with traumatic brain injuries, spinal cord injuries and other needs requiring high level care. Trach and ventilator experience is required. It is a transitional care unit that offers a great opportunity to develop as a manager while gaining hands-on, nursing experience. Work with a collaborative team that includes respiratory and therapy in a supportive environment at a recently updated, state of the art facility.

Responsibilities:
  • Direct and oversee patient care of team members.
  • Act as a patient liaison and advocate.
  • Assure proper documentation utilizing the EMR including documenting assessments, initiating care plans, formulating, updating, and revising treatment plans and goals.
  • Participate in discharge planning including identifying home educational needs and instructing and educating patients for ADLs.
  • Assure orders are processed including checking and signing off orders, accepting telephone orders from physicians and ensuring documentation of advanced directives.
  • Make initial and continuous patient assessment including communicating the interpretation of lab data, EKG’s, and other tests. Consult with physician manage patient’s in-house transfer through system, review drug therapy and coordinate administration of treatments between disciplines.
  • Coordinate all aspects of patient care, including planning, intervention, education, and evaluation including age-specific knowledge, phlebotomy, IV access and staple removal.
  • Initiate physician orders, participate in Code Blue response, and provide patient education.
  • Ensure patient handoff to other disciplines through verbal or written communication.
  • Interpret Facility Policy for team members and participate as committee chair or member of various nursing or interdisciplinary committee meetings as requested.
  • Assist in providing adequate staffing and participate in policy development.
  • Function as a charge nurse, assigning care of patients to caregivers according to policy.
  • Participate in education and staff development through direct learning, teaching, orientation and peer review.
  • Perform duties and responsibilities in a manner consistent with our mission and values.
Qualifications:
  • Current licensure as a RN in the state of Missouri Required.

Registered Nurse (RN)

Speech Language Pathologist

Under the supervision of the Director of Rehabilitation, the Speech Language Pathologist implements high quality speech-language pathology services for patients, in accordance with the state regulations, principles and practices of speech-language pathology and HealthBridge policies and procedures.

Qualifications:
  • Master’s Degree in Speech-Language Pathology
  • Current American Speech-Language Hearing Association (ASHA) Certificate of Clinical Competency (CCC) in Speech-Language Pathology or current eligibility for the certification process by following ASHA guidelines for the Clinical Fellowship Year experience.
  • Valid state license
  • Strong background in brain injury rehabilitation desired
  • Vent/trach experience preferred
  • Excellent communication and documentation skills
